SHSMD has agreed to keep individual assessment responses confidential as described below.

• Survey participants are connected to the system through a secure login process. No SHSMD employee has or will have access to your personal information tied to your responses.
• Participant information related to where you work and your role in the organization and other demographic variables will be matched to your assessment responses for aggregate reporting purposes only. You will not be identifiable from your personal assessments responses.
• Your results will be visible to you; no SHSMD employee will have access to your results.
• Only aggregated results from the assessments will be used by SHSMD to improve our programs and services.
